Request changes to the class schedule 

Class Section Edit Request

Request an edit to or cancellation of an existing class section for the current or a future term.  

Section Creation Request

Request to add a section of an existing course to the class schedule for the current or a future term. Use this form to request Individual Studies.

Spring semester Classroom request

Request a general-inventory classroom for your Spring semester classes. This form is available in October and November for the upcoming Spring semester. To request to change an assigned classroom, use the form below.

Room Change Request                           

Request a classroom change for an existing class section for the current term. (This form is only available one month before the start of term through the first two weeks of the term. At all other times, email curriculum@conncoll.edu).

Final Exam Forms
 
Final Exam Information Collection To inform the Registrar's Office if you are offering a scheduled or self-scheduled exam in your class(es) - available during the call for exams.
Self-Scheduled Exam Copy Submission To submit a master copy of any self-scheduled exams. (*only available for current term; two-three weeks prior to the exam period).

IQ Curriculum Management Forms (log in required)                                          

Proposed changes for existing courses or proposals for new courses must be submitted through the IQ Curriculum Management System (available on CamelWeb/ For Faculty) to be reviewed by The Academic and Administrative Procedures Committee (AAPC) and, if needed, the Faculty, before they can be listed in the catalog.
ConnCourse Form When proposing a ConnCourse, please download this form, answer the questions, and upload your responses to the IQ form you are using to propose the course. There is a button on the IQ form for "ConnCourse Form Upload."
W Course Form When proposing a W Course, please download this form, answer the questions, and upload your responses to the IQ form you are using to propose the course. There is a button on the IQ form for "W Course Form Upload."

Early Warning Referral (log in required) To alert a class dean about a student who is not making satisfactory academic progress, please use the form found in CamelWeb in My Students.
End of Semester Academic Report for Students of Concern (log in required) To alert the Dean of the College's office regarding an incomplete or low grade, please use the form found in CamelWeb in For Faculty.
